If you have insurance, you must provide the information from your insurance card at the time of scheduling your appointment so that one of our insurance coordinators can verify your insurance coverage prior to your appointment.

We are happy to submit claims to most insurance carriers, although we can only ESTIMATE insurance coverage based on the information supplied by your insurance carrier. Therefore, you, the account holder, are responsible for ESTIMATED out of pocket expenses including deductibles and co-pay at the time of service AND all unpaid balances after insurance has paid, or determined otherwise.
