PREPARE FOR YOUR  visit

Merheb Surgical Arts in Peoria & Pekin IL

Please assist us by preparing the following for your appointment:

• Your surgical referral slip and any X-rays, if applicable
• A list of medications you are presently taking
• Any medical or dental insurance cards
• Your ID
• Please fill out the Online Registration or print the New Patient Paperwork to bring with you
• Arrive 15 minutes prior to your scheduled appointment
• Please inform our office if you take a pre-medication prior to dental work
• IF IV SEDATION IS DESIRED FOR YOUR APPOINTMENT, ADDITIONAL INSTRUCTIONS:
  • Nothing to eat or drink for 8 hours prior to your procedure
  • Have a driver available
  • Wear a short-sleeved, loose-fitting shirt
  • If you take blood pressure medication, please take it with a sip of water

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
